If you do not want to have your information shared, called ‘Opting Out’, you only need to complete a short form and send it directly to the Hampshire Health Record Operational Team with proof of identity and place of residence.
To withhold your medical record from the Hampshire Health record please complete the form attached below and return it to:

You will also have to provide a photocopy of two or more of the following showing your:
- First Name.
Last Name.
Address.
Date of birth.
Accepted forms of identification are:
Current UK Driving Licence.
Current signed passport.
ID Card.
Birth Certificate.
plus one of the following:
Recent Utility Bill. (Within the last three months)
Local Authority Council Tax Bill.
Bank/Building Society Statement of personal account.
If this information is not provided we cannot process this application any further.
Alternatively you can contact your GP directly and ask him to opt you out. If you select this method of opting out please ask your GP to inform the Hampshire Health Record Team so that they can check that your record has been removed from the system.
On opting out of sharing an electronic health record all your clinical details are removed and all that remains is a message confirming that you have declined to share an electronic health record.

Once you have 'Opted Out' your record is completely deleted from the Hampshire Health Record, although the original record in the hospital or your GP surgery remains.
If, at a later date, you decide that you do wish to have a shared record in Hampshire, or you wish to ensure that your information is available, then you will need to complete a further short form and send this directly to the Hampshire Health Record Operational Team.
In either case, we will ensure that your request is carried out as soon as possible.
