How to view your record

Under the Data Protection Act, you have the right to have a copy of your record. If you want to see it you should download the ‘view your record form’ below and return your completed form to the Hampshire Health Record Office at:

    SO50 5PB 

    You will be required to provide identification and proof of address by enclosing a photocopy of two or more of the following showing your:
  • First name.
  • Last Name.
  • Address.
  • Date of birth.

Acceptable forms of identification/proof of address are:

  • Current UK Driving Licence.
  • Current signed passport.
  • ID Card.
  • Birth Certificate.
Plus one of the following:

Recent utility bill. (Within the last three months)
Local Authority Council Tax Bill.
Bank/Building Society statement of personal account.

If this information is not provided we cannot process this application any further.

There may be times when we are required not to show you parts of your record, but this will only happen if there is information that might relate to another patient, or if a health care professional considers that seeing the information might be harmful to you. You will have the opportunity to view and discuss your record with a healthcare professional who will be able to explain any jargon or technical details to you.
