- You will be required to provide identification and proof of address by enclosing a photocopy of two or more of the following showing your:
- First name.
- Last Name.
- Address.
- Date of birth.
Acceptable forms of identification/proof of address are:
- Current UK Driving Licence.
- Current signed passport.
- ID Card.
- Birth Certificate.
Plus one of the following:
Recent utility bill. (Within the last three months)
Local Authority Council Tax Bill.
Bank/Building Society statement of personal account.
If this information is not provided we cannot process this application any further.
Please note: If you are making any other request (View Record/OPT OUT) at the same time. You do not have to send in more copies of identification.
