Have you ever had to explain your medical history over and over again to different people? Can you remember the names of all your medication?
The Hampshire Health Record (HHR) contains key information from hospital, general practice, community care and social services. The system makes this available to the staff who care for you and your family wherever they are, making the treatment you receive safer and more accurate. HHR is managed by the South, Central and West Commissioning Support Unit on behalf of health and social care organisations in Hampshire.
HHR is now integrated with the GP out of hours system for North Hampshire. North Hampshire Urgent Care provides the ‘Hantsdoc’ Out of Hours GP services at Basingstoke and North Hampshire Hospital. Hantsdoc is a successful organisation with an excellent track record of looking after patients when their own surgeries are closed.
HHR provides Hantsdoc with easy access to a summary of the patient information to help treat patients when they need urgent care after GP practices are closed and at weekends.
HHR Updates Patient Information Posters
As part of our continuing commitment to ensuring that patients are informed about the Hampshire Health Record in an easily understood way, we have been working with “Carers Together”, who are a local charity that represents patients and their carers in Hampshire.
Carers Together have helped us produce a new set of posters that will be available to GP surgeries and other organisations to inform patients about HHR. There are 3 new posters are entitled:

HHR helps patients at the end of life
Hampshire Health Record (HHR) is proud to release a new module which will permit End of Life information to be shared with providers of care. These providers will be able to input and/or view information about a patient once they have given their consent. The End of Life module will be populated, as much as is possible, straight from the HHR to minimise input time.
